Line up your crosshair
Put your crosshair on the same reference point as in the clip at the moment of the throw — switch to 0.25× and step through it frame by frame. View angles: pitch -44.46°, yaw -168.06°.

Jump and release LMB 0.13 s after the jump, in the air.

The nade flies 7.76 s. The smoke rises about 4.7 m high; the cloud on the 2D map is computed from the level geometry — it flows around obstacles and does not pass through walls.
